Como bioinformático de formación y profesión enfocado en la medicina de precisión, comentaré dos "problemas" que, creo, colaboran en que las expectativas no se hayan cumplido.

La primera es que, normalmente, las ideas de "lo que se podrá llegar a hacer en el futuro usando X" suelen ser exageradamente optimistas. Por ejemplo, es una realidad que el estudio de las marcas genéticas y/o de expresión nos están permitiendo identificar biomarcadores aplicables a la medicina de precisión y, a la vez, dilucidar cuáles son los procesos y estructuras afectadas tras un fenotipo concreto. Sin embargo, la realidad es que conforme avanzamos vamos descubriendo que únicamente la integración y el estudio cada vez más específico de diferentes tipos de datos nos va a permitir obtener una visual real de qué está ocurriendo y cómo podemos tratarlo (como ya digo, enfocado sobre todo a patologías). Y, lamentablemente, el estudio integrado y específico es complejo, caro y no siempre funcional. Por ejemplo, a la hora de estudiar marcas de expreisón diferencial, cada vez se busca un enfoque más centrado en el nivel de célula única, altamente específico, pero limitante a nivel de capacidad computacional y relativamente difícil de aplicar en el ámbito clínico.

Por otro lado, y en relación a ese último punto, existe una limitación en la interacción "biología computacional - clínica". No todo lo que se investiga y se descubre mediante la aplciación de técnicas de biología computacional es aplicable directamente a la clínica, lo que hace que pierda parte del interés a nivel social y económico. Esto, por ende, limita en gran medida el desarrollo científico.

Estamos (en realidad, llevamos ya un tiempo) en una era emocionante en cuanto al desarrollo de la bioinformática. Pero, desde mi área, aún se requiere trabajo en el área de la investigación traslacional.

As a bioinformatics technician focused on precision medicine, I will comment on two "problems" that, I believe, contribute to the fact that expectations have not been met.

The first is that, usually, the ideas of "what we will be doing in the future using X" tend to be exaggeratedly optimistic. For example, it is a reality that the study of genetic and/or expression markers is allowing us to identify biomarkers for precision medicine. At the same time, these markers allow us elucidate which processes and structures are in a particular phenotype. However, the reality is that as we move forward we are discovering that only the use of more integrated and specific approaches will allow us to obtain a real landscape of what is happening and how we can treat it (as I said, focused mainly on diseases). And, unfortunately, the application of integrated and specific approaches is complex, expensive and not always functional. For example, when it comes to studying differential expression marks, we are moving forward to the single cell level, which is highly specific, but limiting in terms of computational capacity and relatively difficult to apply in the clinical setting.

On the other hand, and in relation to this last point, there is a limitation in the "computational biology-clinical" interaction. Not everything that is researched and discovered through the application of computational biology techniques is directly suitable to the clinic, losing some interest at a social and economic level. This, therefore, greatly limits scientific development.

We are (actually, we have been for some time now) in an exciting era in terms of the development in the bioinformatics field. But, from my point of view, work is still needed in the area of translational research.
